This intake form requires attention to detail and takes some time to complete, but it will save you time and expense in the long run, allowing us to move your case quickly and efficiently.
All estate planning packages include the following documents:
Last Will and Testament
Statutory Durable Power of Attorney (Financial)
Medical Power of Attorney
Directive to Physicians (Advance Directive)
Declaration of Guardian in the Event of Later Incapacity
Our estate planning packages are offered at three levels:
Essential Protection — $1,250 (individual) / $1,750 (couple)
Includes the five core documents listed above.
Complete Protection — $2,250 (individual) / $3,000 (couple)
Everything in Essential, plus a Transfer on Death Deed for your homestead, a Family Emergency Binder organizing all your executed documents and key information, and one annual review call.
Legacy Protection — $3,500 (individual) / $4,750 (couple)
Everything in Complete, plus Transfer on Death Deeds for additional properties (up to 3), a beneficiary designation audit for retirement accounts and life insurance, two annual review calls, and priority scheduling for future legal needs. Our firm can also coordinate a referral to a trust specialist if requested.
